In making this application for enrolment, I am aware of the consequences that may arise from providing false, misleading or incomplete information, including the cancellation of my enrolment or the withdrawal of any offer made by Sydney Design School.

I am aware that Sydney Design School is required to submit data sourced from this enrolment form to the National Centre for Vocational Education Research Ltd. (NCVER) as a regulatory requirement. I have read and I consent to the collection, use and disclosure of my personal information (which may include sensitive information) pursuant to the information detailed, and NCVER policies, procedures and protocols published on NCVER’s website at www.ncver.edu.au

I am aware that the information contained on my enrolment form may be used by my RTO or the following third parties for administrative, regulatory, and/or research purposes:

Privacy Notice

Under the Data Provision Requirements 2012, Design Schools Australia Pty Ltd trading as Sydney Design School is required to collect personal information about you and to disclose that personal information to the National Centre for Vocational Education Research Ltd (NCVER).

Your personal information (including the personal information contained on this enrolment form and your training activity data) may be used or disclosed by Sydney Design School for statistical, regulatory and research purposes. Sydney Design School may disclose your personal information for these purposes to third parties, including:
/ School – if you are a secondary student undertaking VET, including a school-based apprenticeship or traineeship;
/ Employer – if you are enrolled in training paid by your employer;
/ Commonwealth and State or Territory government departments and authorised agencies;
/ NCVER;
/ Organisations conducting student surveys; and
/ Researchers.

Personal information disclosed to NCVER may be used or disclosed for the following purposes:
/ Issuing statements of attainment or qualification, and populating authenticated VET transcripts;
/ Facilitating statistics and research relating to education, including surveys;
/ Understanding how the VET market operates, for policy, workforce planning and consumer information; and
/ Administering VET, including programme administration, regulation, monitoring and evaluation.

You may receive an NCVER student survey which may be administered by an NCVER employee, agent or third party contractor. You may opt out of the survey at the time of being contacted. NCVER will collect, hold, use and disclose your personal information in accordance with the Privacy Act 1988 (Cth), the VET Data Policy and all NCVER policies and protocols (including those published on NCVER’s website at https://www.ncver.edu.au/).

We are required to assess the suitability of your chosen course accounting for your skills and competencies. If we believe the chosen course is not suitable for you, we will contact you to discuss prior to enrolment.

Digital literacy: you will be required to access resources by navigating our online platform, manipulate documents including scanning and converting to PDF, and communicate via email. Software courses require a high digital literacy to understand program requirements and procedures.
Numeracy skills: you may be required to complete tasks such as converting measurements.
Literacy skills: a strong written comprehension is required to understand course materials.
Language skills: you may be required to present information and projects in a written or verbal format.

Enrolment Declaration

I understand and agree that, under the National Vocational Education and Training Regulator (Data Provision Requirements) Instrument 2020, Design Schools Australia Pty Ltd trading as Sydney Design School is required to collect personal information (information or an opinion about me) collected from me, my parent or guardian, such as my name, Unique Student Identifier, date of birth, contact details, training outcomes and performance, sensitive personal information (including my ethnicity or health information) and other enrolment and training activity-related information (together Personal Information) and disclose that Personal Information to the National Centre for Vocational Education Research Ltd (NCVER).

My Personal Information (including the personal information contained on my enrolment form and my training activity data) may be used or disclosed by Design Schools Australia Pty Ltd trading as Sydney Design School for statistical, regulatory and research purposes. Design Schools Australia Pty Ltd trading as Sydney Design School may disclose my personal information for these purposes to third parties, including:

• School – if I am a secondary student undertaking VET, including a school-based apprenticeship or traineeship;
• Employer – if I am enrolled in training paid by my employer;
• Commonwealth and State or Territory government departments and authorised agencies, including the NSW Department of Education (Department);
• NCVER;
• Organisations (including the Department) conducting student surveys; and
• Researchers.

Personal Information disclosed to NCVER may be used or disclosed for the following purposes:

• issuing a VET Statement of Attainment or VET Qualification, and populating Authenticated VET Transcripts;
• facilitating statistics and research relating to education, including surveys;
• understanding how the VET market operates, for policy, workforce planning and consumer information; and
• administering VET, including program administration, regulation, monitoring and evaluation.

I may receive an NCVER student survey which may be administered by an NCVER employee, agent or third party contractor. I may opt out of the survey at the time of being contracted.

NCVER will collect, hold, use and disclose my Personal Information in accordance with the Privacy Act 1988 (Cth), the VET Data Policy and all NCVER policies and protocols (including those published on NCVER’s website at www.ncver.edu.au).

The Department may disclose my Personal Information to other Australian government agencies, including those located in States and Territories outside New South Wales.

The above government agencies may use my Personal Information for any purpose relating to the exercise of their government functions, including but not limited to the evaluation and assessment of my training, the determination of my eligibility to receive subsidised training or for any Fee Exemptions or Concessions. My Personal Information may also be disclosed to other third parties if required by law. I also acknowledge and agree that the Department may contact me by telephone, email or post, during or after I have ceased subsidised training with Design Schools Australia Pty Ltd trading as Sydney Design School for the purposes of evaluating and assessing my subsidised training.

/ I have read, understood and agree to the Terms and Conditions on the Sydney Design School website including information regarding VET Student Loans for the Diploma and Advanced Diploma only.

/ If I am under 18 years, I have had a parent of guardian give their permission by signing this form.

/ I have also read the Student Handbook.

/ I understand that I will receive a credit transfer on the units of competency I have achieved while studying a previous course at Sydney Design School.
